Parking
On street parking is available. Please use parking spaces on
the same side of street as 40 Bolivia Street, if possible.
Upon Arrival
At 40 Bolivia Street, walk up the front steps and follow the porch as it wraps around the house to your left. Enter the side door to the waiting room. On your first visit, plan to arrive 15- 20 minutes before your scheduled appointment to complete the necessary forms.

Cancellation Policy
If you are not able to make your appointment, please call to cancel. A 24 hour notice of cancellation is expected or else a $20 cancellation fee will be assessed.
